If you are a broker, bank, or other nominee that held Eligible Champion Securities or Eligible Core Securities during the Recovery Period, for the beneficial interest of a person or entity other than yourself, you may have previously provided a list of names and addresses of such beneficial owners in connection with the Class Actions.
If you did not previously submit names and addresses of potentially eligible Claimants, you are instructed to submit the names and addresses of Potentially Eligible Claimants to the Fund Administrator, no later than ten (10) calendar days from receipt of the Distribution Plan Notice.

Request the needed number of copies of the Distribution Plan Notice and Proof of Claim Form via email to email@example.com. You must mail the Distribution Plan Notice and Proof of Claim Form to the beneficial owners within seven (7) calendar days of your receipt of the packets.
